Academic Records Request Forms, Graduate Study Reference Report Forms, and Disability Resources and Services Forms are available if you need additional copies for your application.   

All forms are available as PDF files readable with Adobe Acrobat Reader version 3.  Adobe Acrobat is available for free from the Adobe web site . (Use your Browser's navigation buttons to come back to this site).

After you have installed Acrobat Reader on your computer, you should download the forms you need.  Adobe Acrobat Reader will automatically open these forms, and you will be able to print them.

Academic Records Form for Graduate Study

• Academic transcripts are required from each institution you attended. The Academic Records Form is downloadable to assist you in requesting transcripts. Your application will not be processed until transcripts from each institution you have attended are received by the department.

Reference Report for Graduate Study

• If the program requires references that will be used to make admissions decisions, complete and send the form to the individual who will be providing your reference.

Disability Resources and Services (optional)

• If you have a medical or physical impairment that may require some accommodation for your successful matriculation at Temple, the Office of Disability Resources Services has provided a form to assess your potential needs.
